Timing and Energy Conservation Strategies

Dr. Galvez-Jimenez recommends approaching intimacy like any other activity that requires energy: Choose a time when your body is most cooperative, reduce unnecessary muscle work, and rest before you become exhausted.

Here are some strategies that may help.

Work with your strongest time of day. Intimacy doesn’t have to wait until bedtime if that is when your energy is lowest. Pay attention to whether mornings, afternoons, or periods after rest tend to be better for you.

Notice when your medication helps most. Amifampridine (Firdapse), a first-line therapy for LEMS, reaches its highest blood concentration relatively quickly, about 30 to 60 minutes after a dose. Keep track of when you usually feel strongest and when possible, try to plan intimacy within that window, says Galvez-Jimenez. Don’t take an extra dose or change your medication schedule without your prescriber’s approval.

Let the bed, furniture, or your partner support your weight. Modifying positions can help with comfort and stamina, says Galvez-Jimenez. Positions that allow you to recline or lie on your side may require less work from weakened hip and thigh muscles. The partner without LEMS may also be able to take the more physically active role.

Use pillows and props. Pillows, wedges, bolsters, or rolled towels can support your hips, legs, or back and reduce the effort required to hold a position.

Build in breaks. You can pause, change positions, focus on touching or kissing, or let your partner take the lead while your muscles recover.

Galvez-Jimenez encourages couples not to think of every adjustment as a medical accommodation. “Pillows, props, position changes, and pauses can become part of intimate play and may even add variety to the experience,” he says.
